Primary Job Responsibilities:
- Develop and implement quality assurance programs for Home Health and Hospice to ensure compliance with regulatory standards.
- Conduct regular audits and evaluations of care delivery to identify areas for improvement.
- Create and deliver educational programs for nursing staff on best practices and updates in care standards.
- Collaborate with clinical teams to analyze feedback and improve patient care processes.
- Stay current with trends and regulations in home health and hospice care.
- Serve as a resource for staff, providing guidance on clinical practices and quality improvement initiatives.
- Prepare reports on quality metrics and compliance findings for leadership review.
- Develop action plans based on audit findings to enhance patient care delivery.
- Encourage a team-oriented approach to problem-solving and innovation in quality care.
- Current RN license in good standing to practice in Pennsylvania.
- Bachelor's degree in Nursing (BSN) required; Master's degree preferred.
- 5+ years of nursing experience, with at least 2 years in a leadership or quality assurance role.
- Experience in home health and hospice care is essential.
- Strong understanding of regulatory requirements and quality assurance standards.
- Proficient in developing educational materials and teaching methodologies.
- Excellent communication and interpersonal skills.
- Strong analytical skills and experience with data analysis.
- Ability to work collaboratively within a multidisciplinary team.
- Commitment to continual learning and professional development.
- Proof of COVID-19 Vaccination or an approved Medical or Religious exemption.
