Primary Job Responsibilities:

• Develop and implement quality assurance programs for Home Health and Hospice to ensure compliance with regulatory standards.
• Conduct regular audits and evaluations of care delivery to identify areas for improvement.
• Create and deliver educational programs for nursing staff on best practices and updates in care standards.
• Collaborate with clinical teams to analyze feedback and improve patient care processes.
• Stay current with trends and regulations in home health and hospice care.
• Serve as a resource for staff, providing guidance on clinical practices and quality improvement initiatives.
• Prepare reports on quality metrics and compliance findings for leadership review.
• Develop action plans based on audit findings to enhance patient care delivery.
• Encourage a team-oriented approach to problem-solving and innovation in quality care.

• Current RN license in good standing to practice in Pennsylvania.
• Bachelor's degree in Nursing (BSN) required; Master's degree preferred.
• 5+ years of nursing experience, with at least 2 years in a leadership or quality assurance role.
• Experience in home health and hospice care is essential.
• Strong understanding of regulatory requirements and quality assurance standards.
• Proficient in developing educational materials and teaching methodologies.
• Excellent communication and interpersonal skills.
• Strong analytical skills and experience with data analysis.
• Ability to work collaboratively within a multidisciplinary team.
• Commitment to continual learning and professional development.
