KeystoneCare is actively searching for a Full Time Physical Therapist to join our Home Care team in the Philadelphia area. This position provides an incredible opportunity to deliver high-quality rehabilitation services directly to patients in their homes, allowing for a personalized and impactful care experience.

The successful candidate will be responsible for evaluating, planning, and implementing therapeutic programs aimed at restoring functional capabilities and enhancing patients' quality of life. You will work closely with individuals of varying ages and conditions, developing tailored treatment plans based on physician recommendations and individual needs. Collaboration with interdisciplinary teams is essential in ensuring comprehensive patient care and facilitating optimal outcomes.

As a home care Physical Therapist at KeystoneCare, you will be integral to the recovery and well-being of our patients, helping them achieve their rehabilitation goals in a supportive environment.

  • Graduate of an accredited Physical Therapy program
  • Current Pennsylvania Physical Therapy License
  • Minimum of two years experience in Home Care or community-based settings
  • Valid Driver’s License and a reliable vehicle
  • Proof of Auto Insurance
  • Excellent communication skills for collaboration with patients, families, and the care team
  • Strong organizational skills and ability to manage caseload effectively
  • Influenza (Flu) Vaccination or an approved Medical or Religious exemption required
Experience

Physical Therapy: 2 years (Required)

License/Certification:

PA Physical Therapy License (Required)
