We are seeking a compassionate and caring Home Health Aide to join our dedicated team for private duty assignments in the Flourtown, PA area. The Home Health Aide plays a vital role in providing personal care assistance and home support to elderly or ill patients, helping them maintain a clean and safe environment within their own homes. Home Health Aides are responsible for interacting with clients, family members, and healthcare personnel, ensuring clear communication and support as directed by a Registered Nurse and Staffing Coordinator.

Schedule: Full-time, varying shifts

Area: Flourtown, PA

Job Responsibilities
  • Providing frail, ill, or immobile clients with assistance, company, and comfort.
  • Executing the patient care plan and maintaining clear communication with RN Case Manager.
  • Monitoring the physical, mental, and emotional conditions of clients and reporting any changes.
  • Performing personal hygiene care and assisting clients with daily living activities.
  • Performing light housekeeping duties in the patient's living area.
  • Preparing meals and assisting clients with eating and drinking.
  • Assisting clients with incontinence products and bathroom visits.
  • Running errands and shopping for groceries and basic household items.
  • Assisting clients in moving from beds to chairs or wheelchairs.
  • Accompanying clients on medical appointments with prior approval from the Staffing Coordinator.
  • Maintaining a clean and safe environment.
  • Documenting care using the Agency approved Electronic Visit Verification system.
  • Communicating needs for scheduling changes to the Staffing Coordinator.
  • Administering medicine and treatments as prescribed by healthcare providers with prior authorization and training.

  • Ability to read, write, and communicate effectively.
  • Ability to understand both written and verbal directions and instructions.
  • Certificate or proof of attendance and curriculum of an approved training program or verifiable prior experience as a Home Health Aide.
  • Successful completion of a job-specific knowledge exam.
  • Strong organizational skills and attention to detail.
  • Understanding, compassionate attitude toward the care of the sick.
  • Ability to document actions and observations accurately.
  • One year home health experience preferred.
  • Proof of Influenza Vaccination or an approved Medical or Religious exemption.
  • A valid driver's license and auto insurance if designated to travel from site to site.
